How to Submit Your Financial Aid Forms

  • All forms must include your physical signature — we cannot accept electronic signatures of any kind.
  • Do not email in your forms. We cannot accept emailed forms.
  • Do not submit photo images of your forms. Submitting forms as photo images can make it slower for us to process your forms.
  • If you need access to a scanner, you can download a free app called CamScanner. CamScanner will create Adobe PDFs documents for you.

Submit Financial Aid Forms Through MyCollege (Recommended)

Submit your completed forms to our office by uploading them to MyCollege:

  1. Log in to your MyCollege.
  2. Select Student Services.
  3. Select Financial Aid Forms.
  4. Select the Financial Aid Upload form.
  5. Upload your form.

If you are not able to upload your documents to MyCollege, you can still bring them in-person to a Financial Aid office at any campus.

    2023-24 Forms

    Use 2023-24 forms for fall 2023, spring 2024, and summer 2024.

    Instructions for Forms Submitted through DocuSign

    Some forms for the 2023-2024 academic year can be submitted through DocuSign. DocuSign allows you to quickly and securely sign documents electronically and eliminates the need to deliver traditional paper forms. Central Piedmont students are able to complete and sign documents for the forms that are available through DocuSign.

    If an additional signer is required, such as a parent, you will have the option to provide the email address where DocuSign can forward the document for a signature.

    Email validation is required for all forms. Once the document is signed, the form will be routed back to the Financial Aid Office and will attach to your student account. Please allow 1-3 days for your checklist to reflect completed forms.

    If the additional signer does not have an email address, there is a print version for each of the forms in the following list.
