The request for student’s taxpayer identification number form (PDF) will allow students to submit their social security number or individual taxpayer Identification number to Central Piedmont so that their 1098-T form can be filed. Penalties for Failure to Furnish Correct Social Security Number
Failure to furnish correct social security number. If you fail to furnish your correct social security number or tax identification number to Central Piedmont Community College, the IRS may impose a penalty of $50 unless your failure is due to reasonable cause and not to willful neglect.
Penalties for Misuse of Social Security Numbers
If Central Piedmont Community College discloses or uses your social security number in violation of Federal law, Central Piedmont Community College may be subject to civil and criminal penalties.
