Effective July 1, 2011, individuals interested in the Nurse Aide I, Phlebotomy, EMT, or Dietary Managers programs will be required to take the ACCUPLACER Reading and Arithmetic tests. Please complete this form and bring to the Cato, Central, Levine, or North Campus Testing Center.
