These are pdf files containing evaluation for your Human Services Courses. Please use these forms for your HSE, SAB and DDT course evaluations. Print these files, fill them out and give them to your Program Chair. If you are an SAB Student, please give it to Ms. Bettie Dibrell. If you are a Human Services Student, please give it to Mr. Tommy Lopez. If you are a Developmental Disabilities Student please give it to Ms. Kay Miller.
