This is the final step in the process. Once the research analysis has concluded and this form has been completed, a final recommendation must be made. Based on the research, should this program idea advance to the next phase in the process?

Once complete, print the official form and discuss it with your program supervisor. Your program supervisor may then sign and date the form. The program supervisor should then take the form to your area dean for consideration. Upon review and approval, your dean may then sign and date the form. The dean may then proceed to bring the form to the VP of Learning. Upon review and approval, the VP may sign and date this form. All three signatures are required in order for this program idea to progress to the next phase in the process to develop a new program.
