Lab Reporting - Curriculum & ALC

  • Student's name
  • Social security number
  • Curriculum program code (this is found in the fourth column of the 10% roster).
  • Student's signature
  • Class name, number, and section
  • Days and hours student attended
  • Total of all hours student attended.** Round off totals on each card to the nearest quarter hour.
    .25 = 15 minutes
    .50 = 30 minutes
    .75 = 45 minutes
  • On each card, circle the total and initial it.
  • Referral slip from instructor.
**If a student has withdrawn, we can still count the hours the student attended.

How to prepare the cards for FTE processing

  1. Separate the cards by curriculum program codes.
  2. Alphabetize the cards by student’s last name (all students in this group will have the same program code).
  3. On the 10% roster, record the total for each program code with fractions and then round off to the nearest whole number.
  4. Example:
    110.39=110
    235.5 = 236
    556.67 = 557
  5. Add all totals (whole numbers) to get the total contact hours for the entire section, record the grand total beside "Contact Hours",  and record the total number of students counted  beside "Total Students Enrolled At Reporting Date" (both are above the students names).
  6. Staple or rubber band each group of identical program codes together.
  7. Wrap the roster around the cards.
  8. Turn this information in to Earonita Strong – Compliance & Audit, College Administration Bldg. #4.
