Medical Insurance Coverage - UPDATE
Students:
As you are aware, all students must carry personal medical insurance to be enrolled in any health careers program at CPCC. You must turn in verification of medical insurance, in the form of an insurance card, to your faculty advisor every semester and again prior to beginning clinical rotations (2nd short session of Spring Semester). Faculty advisors will need to keep a copy, which you provide, of your insurance card, at each submission date.
PLEASE NOTE: The insurance card must have your name on it as the subscriber or beneficiary of the policy. In addition, it must list the coverage dates (beginning and end). If it does not have this information, you will need to secure a letter from your insurance company verifying this information.
Please turn the copy of your insurance card or letter of insurance verification and coverage dates in to your faculty advisor by:
1) August 15th,
2) at the beginning of Spring Semester, (1/10/13)
3) at the beginning of clinicals in March, 2013 please turn in to your faculty advisor and/ or Clinical Supervisor ,
4) and at the beginning of Summer Semester (mid-May, 2013)!
