Information about late withdrawals due to a medical or disability issue

It is important for students to understand that CPCC does not have a “Medical Withdrawal” policy.  All withdrawals from any class will be recorded on a student’s transcript as a “W,” regardless of the circumstance that caused the student to withdraw.  If a student is receiving Financial Aid, the student should contact FA to learn how the withdrawal could impact their financial aid award. Late withdrawals from a class are permitted at the discretion of the instructor.  CPCC will not give a refund for a late withdrawal.

 

Procedures for late withdrawals

Notify your instructor about your situation. Disability Counseling Services will, if appropriate, verify the fact that you need to withdraw from class due to a legitimate medical or disability concern.

Talk to a Disability Counselor.  The Counselor will talk to you about your current situation and ask you to sign a FERPA form, which will give us your written permission to discuss your situation with faculty and staff at CPCC.

Provide documentation about your medical condition and your reason for needing to withdraw from class(es) to Disability Counseling Services.

The diagnosis must be made by a licensed health care worker who has training and relevant expertise in the area the diagnosis is being made. The report must include the name, title, and credentials of the qualified professional writing the report.

The documentation should include a clear statement of health impairment along with DSM-IV-TR or ICD Codes, expected prognosis and/or stability of health condition overtime, and a description of the assessment procedures and evaluation instruments used to make the diagnosis, including evaluation results and standard scores, if applicable.

Documentation should also describe the impact the diagnosis has on the student in a post-secondary environment.

Disability Counseling Services will review your documentation once it has been received, and will contact you to further discuss your circumstances.  If the documentation provided is appropriate, the Disability Counselor will contact your instructor by e-mail to inform them of your situation.  However, it is the decision of your instructor(s) to allow for a late withdrawal from class(es).
